History of NY Connects: Choices for Long Term Care
A Restructured System
The New York State Office for the Aging ( NYSOFA) and Department of Health (DOH) are working collaboratively to fulfill a vision of a restructured and accessible long term care system that supports the consumer’s choice and independence and provides affordable services that meet their individual needs.
NY Connects: Choices for Long Term Care is a first step toward achieving that vision. It is a State-funded program to establish county level, consumer-centered access points to information and assistance for all individuals in need of long term care services, regardless of age, income, or payment source.
